Apply
If you received an application packet in the mail or have been nominated to apply for the program, you must complete an application. You can filll out the application you received in the mail OR you can complete the application electronically.
There are two ways to fill out the application electronically:
Microsoft Word
Forms can be completed electronically then be saved and e-mailed as an attachment to hscarter@ufl.edu. You must have Word installed on your computer to use these files.

Adobe PDF
Forms can be completed electronically and printed. You must have Adobe Reader installed on your computer to use these files. Download the free software from Adobe.
